Zip Lining at Cypress Valley Canopy Tours in Spicewood, TX (between Austin and Marble Falls). I need the following by Monday July 6 from everyone who plans to attend (so I can obtain the necessary tour permit and also a $10 per person pre-payment discount):

• Completed Permission Slip
• Completed Medical Form parts A and C only (good for one year)
• Completed Canopy Tour Participant Agreement
• Check for $55 (made out to Venturing Crew 505)
